Costo económico y criterio técnico para la contratación de personal: el caso del Servicio de Enfermería, Hospital Nacional Psiquiátrico 2011-2018

Resumen
La presente investigación se desarrolla en el Hospital Nacional Psiquiátrico,
perteneciente a la Caja Costarricense del Seguro Social, desde su origen en 1973
como Hospital Nacional Especializado. Dicha infraestructura corresponde a
establecimientos que responden a una necesidad específica dentro del sector
salud, cuya ubicación es en el Valle Central. (EcuRed Contributors , 2019)
La ejecución presupuestaria de la CCSS plantea 3 escenarios en la
práctica, sub ejecución (debajo del 99%), ideal (que alcance el 100%), sobre
ejecución (sobra pasa el 100%). Por ende, es sumamente importante tener un
estricto control y seguimiento de la ejecución del presupuesto, en aras de cumplir
las metas y objetivos trazados en la formulación presupuestaria, con los recursos
asignados.
Este trabajo se plantea como objetivo general realizar un análisis
comparativo entre el costo económico y el criterio técnico de la Jefatura del
Servicio que implica contratar personal sustituto (a nivel interno de la Institución)
en la partida “2002” versus la cancelación de tiempo extraordinario en la partida
“2021”, en el periodo comprendido entre el año 2011-2018, en la Dirección de
Enfermería; en los puestos técnicos categorizados como asistentes de pacientes,
auxiliares de enfermería y enfermeros profesionales 1, en el Hospital Nacional
Psiquiátrico de la Caja Costarricense de Seguro Social.
El objetivo es analizar la ejecución y el comportamiento de las partidas
presupuestarias de personal sustituto y tiempo extraordinario en el periodo de
estudio. La presente investigación se estructura en tres capítulos, primero se
realiza un diagnóstico de la ejecución presupuestaria de la partida 2021 tiempo
extraordinario y 2002 personal sustituto en el periodo 2011-2018”. En este
apartado se presenta la ejecución presupuestaria y comportamiento de ambas
partidas en la etapa establecida, para una mejor compresión del tema análisis. Segundo, se analizan los costos que conlleva el pago de tiempo
extraordinario o bien contratar personal sustituto. En esta sección se exponen los
resultados de un análisis de costos proyectada en una escala de 15 años de
antigüedad de un funcionario más otros incentivos que son cancelados, ya sea
cuando se contrate personal sustituto o bien se cancele tiempo extraordinario.
Tercero, se presentan los resultados de una entrevista dirigida a la Jefatura
del Servicio sobre el criterio técnico que fundamenta el pago de tiempo
extraordinario o la contratación de personal sustituto. Esto con el fin de conocer el
fundamento que respalda la decisión de optar por alguna de las figuras que están
en objeto de estudio.
Finalmente, las conclusiones y recomendaciones obtenidas del presente
trabajo de investigación, que tiene como propósito brindar alternativas para
mejorar la eficiencia y eficacia de la ejecución de estas partidas presupuestarias
en pro de brindar servicios de calidad a la ciudadanía costarricense, mediante una
ejecución presupuestaria adecuada.
The present investigation is developed in the National Psychiatric Hospital, belonging to the Costa Rican Social Security Fund, since its inception in 1973 as a National Specialized Hospital. This infrastructure corresponds to establishments that respond to a specific need within the sector health, whose location is in the Central Valley. (EcuRed Contributors, 2019) The budget execution of the CCSS poses 3 scenarios in the practice, sub-execution (below 99%), ideal (reaching 100%), above execution (overpasses 100%). Therefore, it is extremely important to have a strict control and monitoring of budget execution, in order to comply the goals and objectives outlined in the budget formulation, with the resources assigned. The general objective of this work is to carry out an analysis comparison between the economic cost and the technical criteria of the Headquarters of the Service that implies hiring substitute personnel (internally of the Institution) in the item “2002” versus the cancellation of overtime in the item "2021", in the period between 2011-2018, in the Directorate of Nursing; in technical positions categorized as patient assistants, nursing assistants and professional nurses 1, at the National Hospital Psychiatric Hospital of the Costa Rican Social Security Fund. The objective is to analyze the execution and behavior of the games budget for substitute personnel and extraordinary time in the period of study. This research is structured in three chapters, first carries out a diagnosis of the budget execution of the 2021 item time extraordinary and 2002 substitute personnel in the period 2011-2018”. In this section presents the budget execution and behavior of both games in the established stage, for a better understanding of the analysis topic. Second, the costs involved in paying time are analyzed. extraordinary or hire substitute staff. In this section, the results of a cost analysis projected on a scale of 15 years of seniority of an official plus other incentives that are cancelled, either when substitute staff is hired or overtime is canceled. Third, the results of an interview directed to the Headquarters are presented. of the Service on the technical criterion that bases the payment of time extraordinary or the hiring of substitute personnel. This in order to know the foundation that supports the decision to opt for any of the figures that are under study. Finally, the conclusions and recommendations obtained from this research work, whose purpose is to provide alternatives for improve the efficiency and effectiveness of the execution of these budget items in favor of providing quality services to Costa Rican citizens, through a adequate budget execution.
