Pasantía en medicina interna y cirugía de tejidos blandos de especies menores en el Centro Médico Animal, La Guácima, Alajuela y en el Hospital Veterinario Hipermascotas, Guadalupe, Goicoechea

Abstract

La medicina interna se enfoca en el diagnóstico, tratamiento no quirúrgico y prevención de enfermedades, y permite indagar más mediante pruebas complementarias para proponer diagnósticos diferenciales e idear un plan terapéutico. Por otro lado, existen casos donde se necesita una intervención quirúrgica complementaria, para lo cual existen cirugías de tejidos blandos y ortopédicas. Cualquiera que sea el tipo de abordaje debe haber un entendimiento mutuo entre propietario y veterinario que promueva la toma de decisiones informadas. El objetivo principal de este Trabajo Final de Graduación fue potenciar nuevos conocimientos y habilidades en el manejo de caninos y felinos en medicina interna y en cirugía de tejidos blandos. Para esto se realizó una pasantía en el Centro Médico Animal (CMA) en la Guácima, Alajuela y en el Hospital Veterinario Hipermascotas (HH) en Guadalupe, Goicoechea entre enero y marzo de 2025: se completaron 188 horas en cada lugar y se asistieron consultas, cirugías de tejidos blandos y pacientes internados. Se atendió un total de 258 casos en el CMA y 186 casos en el HH. Un 87% (n=225) fueron casos de medicina interna sin cirugía en el CMA, 8% (n=20) de cirugías, 4% (n=10) de casos de medicina interna con cirugía y 1% (n=3) de eutanasias. En el HH se abordó un 84% (n=157) de casos de medicina interna sin cirugía, 7% (n=12) de cirugías, 6% (n=11) de casos de medicina interna con cirugía y 3% (n=6) de eutanasias. En el CMA las principales áreas de medicina interna fueron gastroenterología (21.3%, n=50), dermatología (20.9%, n=49) y medicina preventiva (18.3%, n=43); en el HH fueron gastroenterología (17.3%, n=29), nefrourología (16.7%, n=28) y dermatología (10.1%, n=17). Se realizaron 150 pruebas complementarias en el CMA y 298 pruebas en el HH. Las cirugías más comunes fueron: ovariohisterectomía (40.0%, n=12) y nodulectomía (13.3%, n=4) en el CMA; profilaxis dental (13.0%, n=3) y profilaxis dental con nodulectomía (13.0%, n=3) en el HH. Se describieron diversos tipos de abordajes de medicina interna en ambas veterinarias. Además, se aplicaron y reforzaron las habilidades anestésicas y quirúrgicas durante la asistencia de las cirugías de tejidos blandos. También se reforzaron habilidades blandas de comunicación y empatía al interactuar con el equipo de trabajo, así como con los propietarios de los pacientes.

Internal medicine focuses on diagnosis, non-surgical treatment, and prevention of disease, and allows for further investigation through complementary testing to propose differential diagnoses and develop a therapeutic plan. On the other hand, there are cases where complementary surgical intervention is necessary, for which soft tissue and orthopedic surgeries are available. Regardless of the type of approach, there must be a mutual understanding between the owner and veterinarian that promotes informed decision-making. At the CMA, the main areas of internal medicine were gastroenterology (21.3%, n=50), dermatology (20.9%, n=49) and preventive medicine (18.3%, n=43); at the HH, they were gastroenterology (17.3%, n=29), nephrourology (16.7%, n=28) and dermatology (10.1%, n=17). A total of 150 complementary tests were performed at the CMA and 298 tests at the HH. The most common surgeries were ovariohysterectomy (40.0%, n=12) and nodulectomy (13.3%, n=4) in the CMA; dental prophylaxis (13.0%, n=3) and dental prophylaxis with nodulectomy (13.0%, n=3) in the HH. Several types of internal medicine approaches were described in both veterinary clinics. In addition, anesthetic and surgical skills were applied and reinforced during soft tissue surgery. Soft communication and empathy skills were also strengthened when interacting with the staff, as well as with the patients' owners.
