Medicina interna en caninos y felinos en la Clínica Veterinaria Drs. Robert, San José, Costa Rica

Resumen
Esta pasantía fue realizada en la Clínica Veterinaria de los Drs. Robert, en
Curridabat, Costa Rica, con una duración de ocho semanas, para un total de 344 horas
de trabajo. Se participó en la atención veterinaria de 490 pacientes, de los cuales 471
fueron caninos (96,1%), con 244 hembras y 227 machos,18 fueron felinos (3,7%), con
diez hembras y ocho machos, y un paciente fue un conejo doméstico (0,2%).
El principal motivo de consulta fue la medicina preventiva con 169 pacientes
(34,5%). En esta consulta se incluía la revisión del estado general de salud, examen
directo de heces, desparasitación y vacunación. En segundo lugar, estuvo la consulta
debida a sintomatologías gastrointestinales con 70 pacientes (14,3%), y, en tercer
lugar, la consulta dermatológica con 31 pacientes (6,3%). Además, se atendieron 270
pacientes en consultas en otras áreas como nefrología/urología, reproductivo,
neumología, endocrinología, oncología, oftalmología, y ortopedia, entre otros.
Durante las consultas, se realizaron 184 pruebas complementarias, siendo en
su mayoría radiografías, con 77 exposiciones (41,8%), en segundo lugar, el
hemograma y las químicas sanguíneas con 44 pruebas (23,9%), y, en tercer lugar, se
realizaron 15 ultrasonidos (8,2%). Además, se hicieron otras exámenes como la
prueba de fluoresceína, urianálisis con cistocentesis guiada por ultrasonografía, kit de
diagnóstico rápido para descartar parvovirus en caninos y en felinos el virus de la
inmunodeficiencia felina y leucemia felina, otras muestras fueron enviadas al
laboratorio externo para el análisis mediante PCR en casos sospechosos de Ehrlichia

spp., así mismo, se realizaron dos endoscopías, siendo una de ellas de especial
interés para el desarrollo de un caso clínico durante la pasantía.
Además, se participó en 80 cirugías (16,3%), siendo en su mayoría castraciones
con 26 procedimientos (32,5%) entre hembras y machos de ambas especies, seguido
de 12 remociones de masas tumorales (15%), seis cesáreas (7,5%), seis cirugías de
ligamento cruzado o TTA (7,5%), cinco amputaciones de falange (6,3%), cuatro
suturas de heridas (5%), cuatro cirugías de luxación de patela (5%), cuatro otectomías
(5%), dos endoscopías (2,5%), dos nefrectomías (2,5%), dos remociones de pines
ortopédicos (2,5%), una amputación de extremidad anterior (1,3%), una corrección de
entropión (1,3%), un otohematoma (1,3%), una laparotomía exploratoria (1,3%), una
ablación de conducto auditivo (1,3%), una cirugía de remoción de cálculos en vejiga
(1,3%) y, una cirugía de drenaje de bullas timpánicas (1,3%). También se realizaron
22 limpiezas dentales (4,5%), algunas con extracciones de piezas dentales
deterioradas y de colmillos de leche.
This internship was carried out at the veterinary clinic of Drs. Robert, in Curridabat, Costa Rica. It lasted eight weeks, with a minimum hour count of 344. A participation was done, in the veterinary care of 490 patients, from which 471 were canine (96.1%), with 244 females and 227 males, 18 were feline (3.7%), with ten females and eight males, and one patient was a domestic rabbit (0.2%). The main reason for veterinary consultation was preventive medicine with 169 patients (34.5%). This consultation included a review of the general state of health of the animal, direct examination of feces, deworming and vaccination. In second place was the consultation due to gastrointestinal symptoms with 70 patients (14.3%), and, in third place, the dermatological consultation with 31 patients (6.3%). Also, 270 patients were seen in consultations in other areas such as nephrology/urology, reproductive, pneumology, endocrinology, oncology, ophthalmology, and orthopedics, among others. During the consultations, 184 complementary tests made, according to the client's disposition, being mostly X-rays, with 77 exposures (41.8%), secondly, the complete blood count and blood chemistry with 44 tests (23.9%), and, in third place, 15 ultrasounds (8.2%) were performed. In addition, other tests were performed, such as fluorescein test, urinalysis with ultrasound-guided cystocentesis, rapid diagnostic kit to rule out parvovirus in canines, and feline immunodeficiency virus and leukemia in felines. Other samples were sent to the external laboratory for analysis by PCR in suspected cases of Ehrlichia spp. Likewise, two endoscopies were performed, one of xiv them being of special interest for the development of a clinical case during the internship. In addition, there was assistance in 80 surgeries (16.3%), being mostly castrations with 26 procedures (32.5%) between females and males of both species, followed by 12 removals of tumor masses (15%), six caesarean sections (7.5%), six cruciate ligament or TTA surgeries (7.5%), five phalanx amputations (6.3%), four wound sutures (5%), four patella dislocation surgeries (5% ), four otectomies (5%), two endoscopies (2.5%), two nephrectomies (2.5%), two orthopedic pin removals (2.5%), one forelimb amputation (1.3%) , one entropion correction (1.3%), one otohematoma (1.3%), one exploratory laparotomy (1.3%), one ear canal ablation (1.3%), one stone removal surgery in bladder (1.3%) and, a tympanic bulla drainage surgery (1.3%). Also, 22 dental cleanings (4.5%) were also carried out, some with extractions of damaged teeth and milk fangs.
