Pasantía en medicina interna y cirugía de tejidos blandos en especies menores en la Clínica TAMUX Medicina Veterinaria, San José, Costa Rica

Resumen
Esta pasantía fue realizada en la Clínica TAMUX Medicina Veterinaria, en Escazú, San José, Costa Rica, del 3 de enero al 12 de febrero de 2022, con una duración de seis semanas, para un mínimo de 372 horas de trabajo. Se participó en la atención veterinaria de 573 pacientes, correspondiendo 437 (76,27%) a pacientes caninos, de esos 231 fueron hembras y 206 machos y 136 (23,73%) pacientes felinos, de los cuales 75 fueron hembras y 61 machos.
El motivo de consulta que prevaleció fue la medicina preventiva con 158 pacientes correspondiendo al 27, 57% del total, en esta cita se incluía la revisión del estado general del animal, examen directo de heces por medio del microscopio, desparasitación y vacunación. En segundo lugar, la consulta por problemas gastrointestinales con 93 pacientes (16,23%) y en tercer lugar la consulta dermatológica con 32 pacientes (5,58%). En una cantidad menor se atendieron consultas de otras áreas como medicina general, cardiología, endocrinología, oftalmología, neurología, toxicología, ortopedia, entre otros, para un total de 157 (27,39%) y 37 pacientes (6,45%) que se vieron posteriormente para un control de su tratamiento.
Durante las consultas, se realizaron 240 pruebas complementarias, siendo en su mayoría el examen directo de heces al microscopio con 164 (68,33%) pruebas, en segundo lugar, el hemograma y las químicas sanguíneas con 29 (12,08%) y, en tercer lugar, las radiografías y el ultrasonido con 22 (9,16%). a las mencionadas anteriormente, se llevaron a cabo raspados, hisopados, Kit diagnóstico para descartar principalmente Ehrlichia spp. en caninos y el virus de la inmunodeficiencia felina y leucemia felina, urianálisis, test de Schimer y test de fluoresceína.
Además, se participó en 96 cirugías (16, 75% de los pacientes atendidos), siendo en su mayoría castraciones con 62 procedimientos (64,58%) entre machos y hembras de ambas especies, seguido por sutura de heridas para un total de 11 (11,45%) y diez cirugías oncológicas (10,41%), cuatro ortopedias (4,16%), tres laparotomías exploratorias (3,12%), dos correcciones de entropión (2,08%), una estafilectomía y rinoplastia (1,04%), un otohematoma (1,04%), una piómetra (1,04%) y una cirugía de traumatología (1,04%). Fueron realizadas 26 limpiezas dentales (4,53%), algunas incluyeron extracciones de piezas dentales y se mantuvieron hospitalizados 65 pacientes (11,34%), tanto en observación como en terapia intensiva, con duración variable desde las cinco horas (que es el protocolo mínimo de la clínica), hasta semanas, a lo largo de la pasantía.
This internship was done at TAMUX Medicina Veterinaria clinic, in Escazu, San Jose, Costa Rica. It lasted six weeks and extended from January 3rd to February 12th, with a minimum hour count of 372. An active participation on 573 patients was done, from which 437 (76,27%) were canine patients, and 136 (23,73%) were felines. From the canines 231 were female and 206 males and from the felines 75 were females and 61 males. The main reason for patient care was preventive medicine with 158 patients (27,57%), task that included general animal state, feces exam using a microscope, deworming and vaccination. The secondary reason was gastrointestinal issues with a total count of 93 patients (16,23%) and in third place dermatologic assessment with 32 patients (5,58%). As mentioned, these were the main reasons but also there were minor appointments for other areas like, primary care, cardiology, endocrinology, ophthalmology, neurology, toxicology, orthopedics, with 157 appointments (27,39%) and 37 patients (6,45%) kept under surveillance after the appointment. During the appointments, 240 complimentary tests were made, from which most were feces test under the microscope with a total of 164 test made (68,33%), next were hemograms and blood chemistry, with 29 procedures made (12,08%). And in third place X-rays and ultrasound with 22 total patients (9,16%). Moreover, scrapings, swabs, glycemic index measurements, diagnosis kit to discard Ehrlichia spp. in canines and FIV and leukemia in felines, also urea analisis, Schimer test and fluorescein test. xii Apart from the above mentioned cases, 96 surgeries were performed (16,75% of patients seen), mostly castrations of male and female for both species with 62 procedures (64,58%). Next it was wound suture and oncology surgeries with a total of eleven (11,45%) and ten (10,41%) respectively, four orthopedics (4,16%), three exploratory laparotomies (3,12%), two entropion corrections (2,08%), one staphylectomy and rhinoplasty (1,04%), one otohematoma (1,04%), one pyometra (1,04%) and one trauma surgery (1,04%). Also, 26 dental cleanings (4,53%) which included dental extractions. 65 patients were kept hospitalized (11,34%) both in observation and on intensive therapy with variable duration which ranged from five hours (minimum protocolary time given in the clinic) to weeks.
