Medicina interna en especies menores en el Hospital Veterinario Intensevet y la Clínica Veterinaria Vicovet en San José, Costa Rica.

Resumen
El ejercicio de la medicina interna implica la comprensión de la fisiopatología y la interpretación apropiada de técnicas diagnósticas.
La práctica de esta especialidad es desafiante al abordar enfermedades multisistématicas y crónicas que requieren un entrenamiento intensivo. Con el fin de fortalecer los conocimientos y desarrollar habilidades clínicas en medicina interna de especies menores, se realizó una pasantía en la Clínica Veterinaria Vicovet (CVV) ubicada en Sabanilla, y en el Hospital Veterinario Intensivet (HVI) en la Sabana.
La duración de la pasantía fue de ocho semanas, en un periodo entre el 8 de mayo y el 1° de julio del 2017, son un cumplimiento de 320 horas. Se atendieron 265 casos en caninos y felinos, representados en su mayoría por hembras caninas adultas sin raza definida, quienes recibieron un abordaje médico. El principal motivo de consulta se asoció con el sistema digestivo en caninos, y con patologías infecciosas en felinos.
Además del estudio general de los casos atendidos, se analizó un caso clínico de hipoadrenocorticismo. Esta patología representa un reto diagnóstico y su manejo implica un desafío tanto para el médico como para el propietario, y un gran compromiso de ambos.
Los casos atendidos evidenciaron que para alcanzar un diagnóstico, se debe sintetizar tanto la información provista por la anamnesis, los hallazgos en el examen objetivo general y los resultados de las pruebas colaterales, así como la respuesta terapéutica, principalmente en patologías de elevada complejidad.
The qualified practice of internal medicine involves understanding the pathophysiology and the appropriate interpretation of diagnostic techniques. The qualified practice of this specialty is challenging task, when dealing with multisystemic and chronic diseases, therefore it requires intensive training. In order to get stronger knowledge and develop clinical skills in minor species internal medicine, an internship was performed at the Vicovet Veterinary Clinic (CVV) located in Sabanilla, and at the Intensive Veterinary Hospital (HVI) in La Sabana. The duration of the internship was of eight weeks, in a period between May 8 and July 1, 2017, is a completion of 320 hours. A total of 265 cases were treated in canines and felines, represented mostly by adult female dogs without defined breed, who received a medical approach. The main reason for consultation was associated with the digestive system in canines, and with infectious pathologies in cats. In addition to the general study of the cases attended, a clinical case of hypoadrenocorticism was analyzed. This pathology represents a diagnostic challenge and its management implies a challenge for both the physician and the owner, and a great commitment from both. The cases attended showed that in order to reach a diagnosis, both the information provided by the anamnesis, the findings in the general objective examination and the results of the collateral tests, as well as the therapeutic response, mainly in high complexity pathologies, must be synthesized.
