Pasantía en medicina interna y de urgencias en la Clínica Veterinaria Vicovet, San José, Costa Rica

Resumen
Se llevó a cabo una pasantía en la Clínica Veterinaria Vicovet ubicada en Sabanilla, Costa
Rica. Esta práctica consistió de 320 horas, que se realizaron entre los meses de marzo y
mayo del 2017.
Se participó en el abordaje de 246 pacientes correspondientes a 184 (75%) caninos y 62
(25%) felinos. De todos los casos 226 (92%) fueron de medicina interna y 20 (8%) de
emergencias.
Las consultas del área de medicina interna fueron en su mayoría de medicina preventiva con
61 (27%) casos, seguido de 27 (12%) casos de ortopedia, 22 (10%) casos de
gastroenterología y 21 (9%) casos de dermatología. Las emergencias atendidas fueron en su
mayoría categoría verde con nueve (45%) casos y categoría amarilla con cinco (25%) casos,
y se debieron en mayor cantidad a trauma por atropello con cuatro (20%) casos, seguido de
enfermedades infecciosas con tres (15%) casos.
Además se desarrollaron dos casos clínicos, uno enfocado a medicina interna de un canino
con Síndrome de Cushing y Síndrome Eutiroideo al que se le realizaron gran cantidad de
pruebas diagnósticas (hemograma, químicas sanguíneas como glucosa, nitrógeno ureico,
creatinina, proteínas totales, alanina aminotransferasa, fosfatasa alcalina, medición de
tiroxina libre, prueba de supresión con dexametasona, ultrasonido, radiografía), y otro
relacionado a la atención de emergencias que consistió de un felino con uroabdomen por
ruptura vesical iatrogénica que se corrigió mediante laparotomía exploratoria y un cierre de
la pared vesical con patrones de sutura Gambee modificado y Cushing.
An 320 hours internship was performed at Vicovet Clinical Veterinary located at Sabanilla, Costa Rica. The internship began on March and was completed on May, 2017. Throughout the working period, 246 patients were attended, distributed as 184 (75%) dogs and 62 (25%) cats. Among all the cases 226 (92%) were of internal medicine and 20 (8%) emergencies. Consultations in the area of internal medicine were distributed mainly as preventive medicine with 61 (27%) cases, orthopedics with 27 (12%) cases, gastroenterology with 22 (10%) cases and dermatology with 21 (9%) cases. The emergencies attended were mostly categorized as green with nine (45%) cases and yellow with five (25%) cases, and were in greater amount due to trauma caused by run-over accidents with four (20%) cases, followed by infectious diseases with three (15%) cases. This document develops two cases, one related with internal medicin of a dog with Cushing's Syndrome and Euthyroid Syndrome to which a great amount of diagnostic tests were executed (hemogram, blood chemistries like glucouse, ureic nitrogen, creatinine, total proteins, alanine aminotrasferase, alkaline phosphatase, free thyroxine measurement, dexamethasone suppression test, ultrasound, x-ray); and the other is related to an emergency, a cat with a uroabdomen cause by a iatrogenic bladder rupture that was corrected by exploratory laparotomy and a closure of the bladder wall with the suture patterns modified Gambee and Cushing
